How to use this tool:

1. Record everything you eat or drink, and approximately how much. Whether a single candy or few bites of a dessert, everything contributes to your body chemistry, thus your health.

2. Be as honest as possible. This record is a tool  to help observe patterns, to help make the most important changes first. No guilt! It’s just information – and your honesty is what makes it work : )

3. Keep the form with you and jot down what you eat when you eat it. Trying to remember every detail at the end of the day takes more time and is less accurate.

Bring in your completed form with each visit. Review it ahead of time to see if you notice any patterns.
